Hartford, CT post acute care practice is seeking a BE/BC Family or Internal Medicine physician to join their team. This role provides high quality care to a diverse patient population. This 18-square-mile city is home to 125,000 residents, world-class dining, international cultural attractions, award-winning entertainment venues and some of the largest corporations in the nation.Perform comprehensive patient assessments, including medical histories and physical exams.  Develop, implement, and adjust individualized care plans based on patient needs.  Monitor progress and coordinate care to optimize outcomes.  Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach.  Educate patients and families on health management and preventive strategies.  No cap on productivity income potential.  Dedicated night call coverage for work-life balance.
