Job Description HOME HEALTH AIDE / CERTIFIED NURSING ASSISTANT
Skilled In-Home Care Attendant Reports To: NURSING SUPERVISOR
Position Summary: Provides personal care services under the
direction of the Registered Nurse or Therapist and preforms various
personal care services as necessary to meet all patients' needs.
Responsible for observing of patients', reporting all observations
and documentation of all observations and all care preformed. The
CNA/HHA is assigned to specific clients by the Registered Nurse or
other appropriate professional and performs services for clients as
necessary to maintain their personal comfort. Qualifications: 1.
Successful completion of a formal certification training program
and a written skills test and competency evaluation. 2. Be at least
eighteen (18) years of age. 3. Minimum of six (6) months experience
working in a home care setting - Skilled Home Health Care
Experience Preferred* 4. Demonstrated ability to read, write, and
follow a written Plan of Care. 5. Excellent oral, written, and
interpersonal skills. 6. Reliable means of transportation to/from
all assignments. 7. Vehicle Insured and Valid Drivers License
Required. Essential Functions: 1. Performs simple procedures as an
extension of therapy services under the direction and supervision
of the therapist. a. Range of motion exercises b. Assistance in
ambulation or exercises 1. Performs personal care activities,
including but not limited to: a. Bathing b. Shampooing c. Skin
care/nail care d. Oral hygiene e. Shaving f. Dressing 1. Performs
household services essential to health care at home, including but
not limited to: a. Meal preparation/feeding b. Laundry c. Light
housekeeping d. Assists (by reminders and/or supervision, not
direct administration) with medications that are ordinarily
self-administered under the direction and supervision of the
Registered Nurse (per state nurse practice laws and Agency policy)
1. Reports any observed or reported changes in the client's
condition and/or needs to the Registered Nurse or Therapist.
Follows reporting guidelines in the Care Plan. 2. Documents care
provided and completes the forms required for the client's records.
Completes the appropriate records to document care given and
pertinent observations. Submits documentation per Agency
requirements-at least weekly. 3. Promotes personal safety and a
safe environment for clients by observing infection control
practices, following Agency guidelines, and reporting unsafe
situations to the Supervisor/Case Manager. 4. Demonstrates safe
practice in the use of equipment. Does not use equipment until
orientation about its operation has been provided. Notifies
supervisor of educational needs. 5. Communicates effectively with
all members of the interdisciplinary team through verbal reports,
participation in staff meetings, and team conferences, as
requested. 6. Maintains confidentiality in all aspects of the job.
7. Successfully completes annual skills competency validation. 8.
Attends in-service programs to meet compliance requirements.
Identifies learning needs to Agency supervisor. a. Performs other
related duties and responsibilities as deemed necessary
Physical/Environmental Demands: 1. Lift/push/pull 50-75 lbs on a
frequent basis. 2. Frequent stooping, twisting, turning and
bending. 3. Elevated risk of exposure to infectious processes. 4.
Use of transfer and other household equipment. Classification:
