Job Description:
Job Description Pride Health is hiring a Nurse Practitioner in
Providence, RI! (Contract) Travel between multiple Long Term Care
Facilities located closely together in the East Providence area
Schedule: Monday-Friday (8am-5pm) Contract: 7-8 month contract Pay
Rate: $66-69/hour Specific Duties: - Provide specialized care to
high-risk health plan members - Work as part of an
interdisciplinary team - Conduct comprehensive assessments - Manage
acute and chronic medical and behavioral conditions - Ensure
accurate documentation, including ICD-10 coding - Communicate and
collaborate with the care team - Facilitate advanced illness and
care planning discussions - Offer counseling and education to
patients and caregivers Perks: - No on-call - No weekends - No
holidays required Must have qualifications/experience: - Nurse
Practitioner or Physician Assistant - NP Certified in one of the
following (FNP, ANP, GNP) - Access to transportation - 1 year of
experience (LTC, SNF, Geriatric preferred) Apply for this role with
