Job Description:
SummaryThe Physical Therapist 2 provides an advanced level of
patient evaluations and treatment according to established policies
and procedures to patients of all ages and populations. The
Physical Therapist 2 is able to create or enhance tools and
processes to improve practice. They mentor others in development of
overall practice.EducationThe Physical Therapist II must have
graduated from an accredited Physical Therapy
program.Certification, Registration & LicensureCurrent American
Heart Association Basic Life support for Healthcare Providers
certification, or equivalent course credentialed by American Heart
Association (American Red Cross CPR/AED for Professional Rescuers
and Healthcare Providers) within 30 days of hire.Licensure
required: Current license to practice as a Physical Therapist in
the state of NH.ExperienceRequires 5 years of experience as a
physical therapist. Requires demonstrated experience interacting
effectively and positively with a multidisciplinary team, patients
and others. Must possess knowledge of growth and development across
a diverse age and multicultural population. Requires demonstrated
advanced clinical reasoning, efficiencies and positive support of
Concord Hospital Rehabilitation Services.ResponsibilitiesAdvocates
for development of the Acute Care Rehab Services practice
model.Responsible for creating or enhancing components of clinical
practice for the Inpatient Team with self directed initiatives.Role
models adaptability, flexibility and timeliness in managing
department workload and processesrequired to meet patient care and
department needs.Advocates for and is actively involved in
change.Performs patient evaluations and re-evaluations in
accordance with licensure, scope of practice anddepartment
standards.Develops Assessments for each patient at evaluation and
during treatment.Develops and re-assesses goals for all patients
during their episodes of care.Develops and implements treatment
plans in order to attain established therapy goals.Communication
includes verbal and written documentation compliant with
professional standards anddepartment guidelines.Takes
responsibility for accurate and thorough patient charging as part
of daily charting.Takes responsibility for individual performance
