Job Description:
A company is looking for a part-time RN Case Manager
(Remote).
Key Responsibilities
Conduct patient outreach to improve HEDIS measure compliance and
educate patients on preventative care
Coordinate care with providers and care teams to close care gaps
and assist patients with referrals to resources
Document patient interactions and develop strategies to enhance
HEDIS performance
Required Qualifications
Licensed Registered Nurse in good standing in the State of
Louisiana or Compact license
3+ years of RN Case Management experience
Bilingual skills (English, Creole or French) are a plus
Familiarity with HEDIS measures and quality improvement
initiatives
Ability to meet accreditation and quality standards
